What the Letter Includes:

• Confirmation of medical necessity

• Diagnosis or clinical rationale (as appropriate)

• Prescribed treatment details

• Provider credentials and signature

This documentation can be submitted to your HSA administrator to assist with reimbursement or verification, depending on your plan’s requirements.

Please note: While we provide the necessary documentation, final reimbursement approval is determined by your HSA provider.
